Will all participants please use the booking form and send with cheque payable to UKAMHS, to Alison Johnston. Bookings will not be accepted for this clinic unless accompanied by a cheque for the full amount and posted along with the booking form to Holly Tree House, Hailstone Moor, Northallerton, North Yorkshire DL6 3QS.
